■ What is IUI, and how does it work?

IUI, or Intrauterine Insemination, is a fertility treatment where washed and prepared sperm are placed directly into the uterus using a thin catheter. This procedure bypasses barriers like cervical mucus and reduces the distance sperm must travel, increasing the chance of fertilisation during ovulation.

IUI is ideal for:

  • Couples with unexplained infertility.
  • Mild male factor infertility (low motility or count).
  • Women with mild endometriosis.
  • Single women or same-sex female couples using donor sperm.
  • Couples with issues related to cervical mucus or ovulatory disorders.

IUI is generally a painless and quick procedure, similar to a Pap smear. Some women may feel slight cramping or mild discomfort during or after the procedure, but it is not typically painful and requires no anaesthesia.

Success rates vary depending on factors like age, fertility issues, and the use of fertility medications. On average:

  • Women under 35 have a success rate of 10-20% per cycle.
  • Women over 40 have lower success rates (5-10% per cycle).
    Multiple cycles may increase the cumulative chance of success.

Preparation for IUI includes:

  • Fertility evaluations to determine ovulation timing.
  • Blood tests or ultrasounds to monitor follicle development.
  • Taking prescribed ovulation-inducing medications if needed.
  • Abstaining from ejaculation for 2-5 days before the procedure to ensure optimal sperm count.

Medication is not always required but can improve success rates. Ovulation-inducing drugs, such as Clomiphene citrate or gonadotropins, stimulate egg production and help time ovulation for the IUI procedure.

The actual IUI procedure takes about 5-10 minutes. However, the full process, including sperm washing and preparation, can take 1-2 hours. It is usually done in a clinic, and no recovery time is required afterward.

IUI is a low-risk procedure. However, minor risks include:

  • Mild cramping or spotting after the procedure.
  • A small chance of infection.
  • If fertility medications are used, there is a slightly higher risk of multiple pregnancies (e.g., twins or triplets).

It is often recommended to try 3-6 IUI cycles before considering advanced treatments like IVF. The number of cycles depends on age, fertility diagnosis, and response to previous treatments.

  • IUI involves placing sperm directly into the uterus to meet the egg naturally. It is less invasive and more affordable.
  • IVF (In Vitro Fertilization) involves fertilising eggs and sperm outside the body in a lab and then transferring the embryo into the uterus. IVF is more complex, costly, and often used for more severe infertility cases.

You can take a pregnancy test about 14 days after the IUI procedure. Testing too early may show inaccurate results due to lingering fertility medications or early hormone fluctuations.

Sperm washing removes seminal fluid, debris, and non-motile sperm, leaving only healthy, highly motile sperm for insemination. This process increases the chances of successful fertilization while reducing the risk of uterine reactions.
